- Proof of Immunity (Optional): Alternatively, you can provide written consent to the health department if you've previously had Hepatitis B and have proof of immunity.

Attach a copy of your photo ID (driver’s license or passport) to your application. You must be 18 or older.

Types of Tattoo Licenses in Alaska –

Tattoo Trainee Permit
It requires a tattooing student to receive at least 1,000 hours of theoretical and practical training. The training must be completed within one year from the date of commencement and include:
- A minimum of 150 hours of practical operations broken out by:
- 50 practical operations observed by the student;
- 50 practical operations in which the student participated;
- 50 practical operations performed by the student under supervision, but without assistance.

Standard License
The Standard License stands as the pinnacle, granting full authorization to practice tattooing within the state. If it is your first time getting a tattoo license and you are living in Alaska, you will want to apply for a Standard License.

Temporary License
The board may issue a temporary license to practice Tattooing to an applicant who has completed their training, is qualified to take the examination and has submitted an application. The temporary license allows an applicant to practice under the supervision of a person who is currently licensed in the field of practice for which the applicant is applying.
Note: Temporary licenses are valid for 120 days and are non-renewable. An individual may not receive more than one temporary license for each area of practice.

Temporary Permit
A temporary permit is available for tattoo artists working in a shop approved by the state. They are valid for 6 months from the date of issue. To apply for a temporary permit, you must issue a notarized copy of your license from another state (or copies from a licensed school in your initial licensing jurisdiction).

If you are a guest artist from another state, you can get a 30-day courtesy license and practice in a licensed shop

Online CPR courses are not approved. CPR courses must be hands-on. Contact the course provider to ensure that your CPR course meets the Board's requirements. CPR courses must be offered by the American Red Cross, the American Heart Association, or an equivalent organization, and must be taken in person, not online.

How to Become a Licensed Tattoo Artist in Arizona in 2024
Arizona's tattoo and body art regulations are relatively lax compared to other states. Unlike places such as North Carolina or Oregon, you don't need a specific license or certification to practice tattooing, piercing, branding, scarification, or similar procedures in Arizona.
Arizona law mandates the use of sterilized needles by Tattoo Artists state-wide, ensuring safe practices for all body art procedures. Tattooing individuals under 18 is prohibited without the physical presence of a parent or legal guardian, regardless of cost or location. That's really the only Arizona tattoo law that applies state-wide for all Tattoo Artists.
However, while the state doesn't regulate tattoo shops at the state level, individual towns and counties may have their own guidelines. It's crucial to check with your county's public health department for specific requirements and must adhere to local regulations. Don’t worry we have done that work for you. As of March 2024, Coconino County is the only county in Arizona that regulates the tattoo industry.
In Coconino County, all body art operators need a business license and must register their shop. Every tattoo artist must also hold a license.

Print out the Form and Fill out your personal information, and submit your the application to the Environmental Health Office along with the appropriate fee. Coconino County mandates Tattoo Artists to complete the Hepatitis B vaccination series. You have to submit copies of your vaccination record or fill out the Medical Record Vaccination Status form. Additionally, Blood-borne pathogen training is mandatory, and you must submit proof of completion along with your application.
